PS Form 4056
Recently I stumbled on to PS Form 4056. This form is for carriers that have mounted routes and do not deliver park and loop assignments.
According to PS Form 4056, postal regulations require customer’s to provide and erect at their own expense rural mail boxes which meet official standards. Whenever possible, mail boxes must be located so that carrier’s vehicle is off pavement when serving them. House number must be painted on mail box in numerals not less than one inch. Post must be neat and of adequate strength and size. Approach to the mail box should be hard level surface (gravel, cinder block or stone). Mail boxes must be placed to conform with state laws and highway regulations.

Here are some issues that your mail box might have that you deliver to: Your mail box is not an approved mail box. The door to the mail box needs attention. Mail box must be located so carrier can serve it without leaving the vehicle. Your mail box is not waterproof. Your mail box should be raised ____inches. Your mail box should be lowered _____inches. Your mail box must be located on the right-hand side of the road in the carrier’s direction of travel. The approach to your mail box should be filled and properly graded & kept unobstructed at all times. The approach to your mail box should be kept clear of snow, vehicles, and other obstacles. The Signal Flag needs attention. Your mailbox is too near the road. Your mail box is too far from the road. Your mail box should face the road. Your mail box should be securely fastened to its support. Your mail box should be made level, and the post firmly planted. A new post for your mail box should be provided. The house number must be printed in numerals not less than one inch high on the side of the mail box visible to the carrier as they approach it or on the mail box door if mail boxes are grouped. Your mail box should be painted or one that doesn’t rust. Your house number must be clearly displayed on your house or on your mail box. There is also a segment for other faults that are not listed for you to describe.
Also on this form it states: please help us provide you with better service by correcting these faults by_____________(date) After this date, delivery service may be suspended until the faults are corrected. Your cooperation is greatly appreciated.
PS Form 4056 is available at every post office, if you have a mounted route and have any of these deficiencies that I mentioned above please ask your supervisor for a form and fill it out and like everything else make a copy for yourself.
