Healthplex Dental Plan

    During this Covid-19 pandemic we may have been putting off our own health care issues or even routine scheduled appointments.  One of these health care issues is your teeth.  I know a lot of people are afraid to go to the dentist on a normal basis but now with this Covid who wants to go to the dentist, but it is very important to maintain your teeth for a good quality of life. 

    NALC Br 6000 offers active and retired members a dental plan it is called Healthplex Dental.  In our newsletter the LIMB there is an advertisement for Healthplex Dental Plan.  Healthplex Dental has been offered to the membership for many years but not many of the membership know about it or take advantage of this benefit.

    The price for Healthplex Dental coverage is as follows:  individual - $263.00, couple - $390.00, member/child - $390.00 and family $566.00 for yearly coverage. 

    Dependent children are covered up to their 19th birthday, or up to their 23rd birthday if a full-time student.

  Healthplex dental plan encourages the prevention of dental disease by covering the following services at no cost: Oral exams, initial and periodic X-rays, semi-annual cleanings, fluoride treatments, oral hygiene instructions, oral cancer exams, routine extractions.  There is a $5.00 copayment per person/per visit to your general dentist. 

    Covered services can only be rendered by participating dentists in the Healthplex plan.  Each covered person must select one participating dentist per family to provide general dental services.  These general dentists will provide all covered services according to the Schedule of Copayments.  Many services will be provided at no cost to the member. Others may have small copayments that patients will pay directly to the dentist.  When endodontic, periodontal, surgical, or orthodontic treatment is needed by a specialist, the participating general dentist will refer the case to participating specialists.  Unless otherwise noted, patient copayments will be the same when services are rendered by participating specialists.

  With Healthplex dental there are no claim forms, no waiting period, no maximum benefit levels, no pre-existing condition exclusions and dependent children are covered from age 19 to 23, if enrolled as a full-time student.

    You may request an informational package for Healthplex Dental Plan by asking your shop steward or you can call me at the Branch office.  In that package will be a form to enroll in the plan, Group benefit page with copayment schedule and a link to go to Healthplex website for a list of general dentistry sites from New York including the 5 boroughs, Nassau, Suffolk, Westchester, Upstate New York Counties, New Jersey, Connecticut, Pennsylvania, and Florida.  Also, there will be an envelope with an address where to mail the completed form and payment.

    If you would like to look up online, you can visit Healthplex.com to easily access the most current dental panel, and log on to see your dental coverage and other important and interesting information about your oral health.  In order to locate participating providers, please follow the instructions listed: go to www.healthplex.com, click on “our dentist” to view the most current listing of participating providers available to you, under the “prospective members” section on the left-hand side: click on “Managed care panels”, click on “comprehensive panel”, choose “general practice” or “specialty” and enter either your zip code or city/county and state.  The Healthplex Group # for our plan is GG-246ID.

      To apply for the Healthplex Dental plan just complete the dental application form with a check for the appropriate premium required and return everything to the address listed on the envelope.  Do not mail the application and check to Branch 6000 as this will only delay the process. 

    Remember don’t wait until you have a toothache to think about dental coverage, get it now and go for those periodical cleanings so you won’t get a toothache!
